Pro North Hockey Training Inc. – Waiver of Liability

This waiver must be agreed to by all individuals participating in any program offered by Pro North Hockey Training Inc. If the participant is under 18 years of age, a parent or court-appointed legal guardian (18 years or older) must agree to this waiver on their behalf.

By registering for and participating in a Pro North Hockey Training Inc. program, I (the participant and/or parent/guardian) acknowledge and agree to the following:

I understand that participation in hockey training, athletic development, and related activities involves inherent risks, including but not limited to injury, illness, or damage to personal property. I voluntarily assume all risks associated with participation.

I agree that Pro North Hockey Training Inc., its owners, coaches, staff, and affiliates shall not be held liable for any injury, loss, or damage sustained by the participant while attending or participating in any program. This includes activities conducted at:

• 130 Saunders Road, Unit 15, Barrie, Ontario

• Any other facility where Pro North Hockey Training Inc. programs are offered

I confirm that the participant is physically able to participate and will follow all instructions provided by staff.

I understand that all participants are responsible for wearing appropriate protective equipment relevant to their sport and training activities.

I consent to the participation of the athlete under the terms of this waiver. If signing as a parent or legal guardian, I confirm that I have the authority to agree on behalf of the participant.

I also consent to the use of the participant’s name for administrative or promotional purposes unless otherwise requested in writing.

By completing registration and providing my name, I acknowledge that I have read, understood, and agree to the terms of this waiver.
